Insurance Policy Notice

Please be advised that we do not accept out-of-state Medicaid nor any Molina insurance plans. We are happy to bill any insurance coverage a patient provides; however, it is the patient’s responsibility to verify coverage and network participation with their insurance provider prior to receiving services. We strongly recommend contacting your insurance company directly to confirm your benefits and any out-of-pocket responsibilities. Please watch for signage in our offices, as occasionally we partner with external providers for specialized services. Notification is posted at check-in regarding other providers you may want to verify are in-network with your insurance.
